Html Form Converted To Flash

I have an HTML form that needs to be converted to a flash file so that all of the variables will get passed correctly.
I was able to create a flash form that passed some of the basic variables, but not all of them. (see attached .fla)
Here’s the html version of the form that that I’m trying to duplicate:

You’ll notice that once you submit the above html form, it takes you to a page that looks like the attached file “correct_page_result.jpg”
