Blackberry Application Jde 4.5

Develop an application for the Blackberry using the Blackberry JDE version 4.5 per the attached specification. NetBeans 6.5 also acceptable with 4.5 compatibility also acceptable.
The application is basically a simple form that assists a user in creating a formatted eMail with an attached photo.
