I have a database db1 and table tbl1 with 4 fields: id, name, birthday, country

I want to have the following form that defaulted to display name, birthday, country where ID = 0

and jquery in the “select name” drop down that list of all the name in database and ‘add new student’ as the last option in the drop down .

If add new student is selected, the form will be empty and user can fill in the form and click on save to save the data to tbl1

It is hard coded for 7 pizzas. I need it to initially show one pizza then select add new pizza with a button.

I need tableless CSS. It can keep the same look as it has now.

Right now uses javascript to instantly update the order and total price on the webpage when a new topping or pizza is selected. But does not work with javascript disabled. It must gracefully degrade to minimum functionality using php.

After ordering, order must email to admin, and the cook and the customer and store in database. I need admin control panel to view todays orders and historic orders and to enter manual orders. and email customers.
