Secure Subscribe Page

When people fill in the subscribe, I need all the fields required and it gives them a popup saying it is required. Also the subscribe.php file needs updated to match form inputs. Also the Flight sim used line allows more than 1 checkmark. I have enclosed a couple files to show what I want.
1: email.jpg (I want the message emailed to me similar to this format)
2: subscribe.php (the php file I use that needs the required fields)
3: subscribe.htm (the htm file that goes to the php)
I will not give access to my ftp.
Test before sending to me.
