Psd – WordPress

I need a professional to take a PSD I have and apply it to WordPress. Attached is the sample.
Template is just the first page layout.
Template_page_2 is the layout for the rest of the website.
The boxes to the left will all have there own pages as well as the nav bar at the top. But, this layout will be the same layout through every single page. (The contact form needs to be working but the form underneath doesn’t.)
